Five Steps to Scheduling Your Appointment

 

STEP 1 – READ  “Your Online Consultation” Note: Please Do Not Schedule your appointment until Step One through Three are completed (New Clients), and I have contacted you. Returning clients only need Step Two before proceeding to Step Four (Scheduling).
STEP 2 – Fill out & Submit your Pre-Client Questionnaire & Appointment Policy
STEP 3 – Upload 1-2 recent photo(s), or text 1-2 recent photo(s) to 805-550-7379 without makeup and in natural, even lighting.
STEP 4 – First, while using a laptop or desktop computer, choose your location; San Luis Obispo OR Vancouver, WA.  Availability for your chosen location will appear   (See note below).  Click on the > (arrow) to advance the calendar.  ONLY appointments which are available will appear.  Secure your appointment by using your credit card information on our secure site.  I only see the last four of your card number.
STEP 5 – READ/PRINT OUT your CARE INSTRUCTIONS after scheduling your appointment.  Do not wait until the morning of your appointment.
NOTE: Once you make your appointment, you will receive a text and email confirmation.  KEEP YOUR CONFIRMATION!  Should you need to reschedule your appointment, the link included in this confirmation will be your link to change your appointment.  For any cancellations, please do so with this link. If you have any questions, please text me.  I get to emails once a week. Because of Telemarketers, I just about avoiding my phone.
NOTE: Appointments cannot be made via Smartphones or Tablet. Only Laptops or Desktops. I apologize for the inconvenience. Follow each step in order if you are new to me.  Do not schedule until all steps have been completed.  Fill out and submit everything together. Be sure to include your photo (unless you are a returning client). Thank-you for making this process operate smoothly.

 

Step 1: Your Online Consultation

Get the best out of your Permanent Makeup procedure – understand what to expect and be well prepared.

Step 2: Pre-Client Questionnaire:

Fill out the online form and then “click” submit.

Step 3: Send us a photo

Send one or more current photos of your face without makeup. Your photo should be taken in natural light, close-up, and focused.  Use the online form in Step 3 to email the photos, or text Carolyn. Be sure to include your Full Name.  If you have had previous permanent makeup prior are requesting for Corrective work of any kind, please do not pre-schedule until you have received “approval” from Carolyn.  Thank you for your patience.

Step 4: Book Online:

Before being able to schedule your appointment, have you completed and submitted Steps One through Three?  If not, please do not continue to Step Four until Steps One, Two &  Three have been completed and submitted.  Please go back and complete these steps first. 

After you have scheduled, please PRINT AND READ your Before Care Instructions.  Do not wait until the morning of your appointment as you have missed some very important information and arrive unprepared.
